Black Ink Printing on Front of Folder with a convenient check box to designate either a Physician or Allied Health Professional Credentialing Record. Included in each folder is the Cred-Plus Divider Set. Folder size: 9-1/2" high plus 1/2" tab x 11-3/4" wide; heavy duty pressboard, 1/2 cut top tab, center position, with 2" yellow tyvek expansion. 50 Credentialing Folders per Carton.
