Top Tab Cred-Plus Credentialing Record Unit. Constructed of heavy duty gray pressboard. Blue Ink Printing on Front of Folder with a convenient check box to designate either a Physician or Allied Health Professional Credentialing Record. Included in each folder is the Cred-Plus Divider Set. Folder size: 9-1/2" high plus 1/2" tab x 11-3/4" wide; heavy duty pressboard, 1/2 cut top tab, center position, with 2" dove gray tyvek expansion. 50 Credentialing Folders per Carton.
This Complete Unit Includes: The Top Tab Cred-Plus Folder with the Complete set of Stock Credentialing Dividers, Mediclip Base, Extension Piece And Compressor Bar Fully Installed.
